The Single Assessment Process (SAP) is a key element of the Government’s plans to improve care provision for adults. It is based on the principle that the individual is the best
adviser on their needs, and the assessment process must facilitate the expression of these needs. It is also based on health and social care staff working together to develop an understanding of the individual’s
needs and to develop the care packages most appropriate to their needs.
HBN’s SAP system provides a single intranet-based client assessment database, to which both health and social care staff can have access, and which can link to existing case
management systems. The system provides for mobile working using tablet or laptop PCs - either to work on-line through a wireless connection, or to work off-line with full synchronisation of the database on return
to the office.
Currently the system is installed in Walsall MBC, Walsall tPCT, and Walsall Hospitals Trust.
